Care Manager

Coordinates a person's care and services; the role's clinical requirements depend on the setting.

A Care Manager coordinates the care and services a person receives, making sure their needs are assessed, planned for and reviewed over time. They are the point of connection between the person, their family, support workers and external services such as allied health. Day to day, they develop and update care plans, organise services, manage budgets where relevant, and step in when needs change.

The clinical requirements for the role depend on the setting. In some Aged Care and community programs, a Care Manager may need to be a registered nurse or another registered practitioner because the work involves clinical oversight. In other settings, the role is more about coordination and the person may come from a community services or social work background. Always check what a particular provider or program requires.

For providers, the Care Manager role is central to person-centred care and to meeting quality expectations. It depends on a clear picture of each worker's skills, qualifications and clearances, so the right person is matched to the right support. Keeping that information current and easy to confirm, as a worker's Career Passport does, helps Care Managers build and adjust the right team around each person.
